• Each applicant over the age of 18 is required to submit a $40 application fee. Married applicants only need to submit ONE application fee. Applications will not be reviewed until BOTH applicants have submitted application fees. 
• Please make sure the name for the payment matches the name for the applicant.
• Once your application and fee is received, we will review your application and get back to you as soon as possible.
